Uses
Primary Use:
Chronic stable angina pectoris (when beta-blockers are inadequate or not tolerated)
Also Used For:
Chronic heart failure (NYHA class II–IV with reduced ejection fraction)
Who May Benefit:
Patients intolerant to beta-blockers or needing additional heart rate control
Warnings & interactions
Warnings:
Can cause bradycardia; may worsen conduction disorders; avoid in unstable cardiac conditions
Avoid If:
Acute decompensated heart failure, severe hypotension, sick sinus syndrome, pacemaker-dependent patients (rate <70 bpm requirement unmet)
Interactions:
• Strong CYP3A4 inhibitors (e.g., ketoconazole) ↑ ivabradine levels • CYP3A4 inducers (e.g., rifampicin) ↓ effect • Beta-blockers ↑ risk of bradycardia • Calcium channel blockers (verapamil/diltiazem) contraindicated • Grapefruit juice increases drug concentration
