Primary Use:
Chronic heart failure (NYHA class II–IV)
Also Used For:
Sometimes used in guideline-directed HF optimization (off-label in select cases under specialist care)

Patients with stable chronic HF, reduced EF, and persistent symptoms despite standard therapy
Warnings & interactions
Warnings:
Risk of hypotension, angioedema, kidney dysfunction, hyperkalemia; avoid during pregnancy
Avoid If:
History of angioedema, pregnancy, severe liver disease, or concomitant ACE inhibitor use (within 36 hours)
Interactions:
ACE inhibitors → angioedema risk Potassium supplements → hyperkalemia NSAIDs → reduced kidney function Diuretics → hypotension risk Lithium → toxicity risk
