Uses
Primary Use:
Treatment of rheumatoid arthritis.
Also Used For:
Psoriatic arthritis, ankylosing spondylitis, ulcerative colitis; sometimes used off-label for certain dermatologic conditions under specialist supervision.
Who May Benefit:
Patients with autoimmune diseases who have insufficient response to conventional DMARDs or biologics.
Warnings & interactions
Warnings:
Increased risk of serious infections, tuberculosis reactivation, shingles, blood clots, cardiovascular events, and certain malignancies. Regular monitoring of CBC, LFTs, lipids, and infection status is recommended.
Avoid If:
Active serious infection, severe liver impairment, pregnancy (unless specialist advises), breastfeeding, hypersensitivity to tofacitinib.
Interactions:
Carbamazepine: May reduce tofacitinib levels. • Phenytoin/Fosphenytoin: May reduce effectiveness. • Bosentan: Lowers drug concentration. • Live vaccines: Avoid due to infection risk. • Other immunosuppressants (e.g., azathioprine, cyclosporine): Increased infection risk.
