Warnings & interactions
Warnings:
High bleeding risk, requires INR monitoring, narrow therapeutic index
Avoid If:
Active bleeding, pregnancy (especially 1st trimester), severe liver disease, uncontrolled hypertension
Interactions:
NSAIDs – ↑ bleeding risk Aspirin/Clopidogrel – additive anticoagulant effect Antibiotics (e.g., metronidazole, erythromycin) – ↑ INR Antiepileptics (phenytoin, carbamazepine) – ↓ or ↑ effect unpredictably Vitamin K foods/supplements – ↓ warfarin effect Alcohol – increases bleeding risk or alters INR Amiodarone – significantly ↑ warfarin levels
