Uses
Primary Use:
Treatment of Gastroesophageal Reflux Disease (GERD) and erosive esophagitis.
Also Used For:
Gastric ulcer Duodenal ulcer Helicobacter pylori eradication (with antibiotics) Prevention of NSAID-associated ulcers Zollinger-Ellison syndrome and other hypersecretory conditions

Patients with chronic heartburn Long-term NSAID users Individuals with recurrent acid reflux Patients requiring ulcer healing and maintenance therapy
Warnings & interactions
Warnings:
Long-term use may increase risk of vitamin B12 deficiency, hypomagnesemia, and bone fractures. Use cautiously in severe liver impairment. Prolonged therapy may increase the risk of gastrointestinal infections such as Clostridioides difficile.
Avoid If:
Known hypersensitivity to Esomeprazole or other PPIs. Concurrent use with drugs where PPI interaction is contraindicated according to prescribing information. Severe allergic reaction history to PPIs.
Interactions:
Clopidogrel: Reduces activation → decreases antiplatelet effect and may increase CV risk. Warfarin: May increase INR → higher bleeding risk; monitor closely. Methotrexate (high dose): Delays clearance → increases toxicity risk. Ketoconazole/Itraconazole: Raises gastric pH → reduces antifungal absorption and effectiveness. Atazanavir/Nelfinavir (HIV drugs): Significantly reduces absorption → contraindicated. Diazepam: Inhibits metabolism → increases sedation and drug levels. Phenytoin: May increase serum levels → risk of toxicity; monitor levels. Digoxin: Increases absorption → potential for toxicity (rare). Iron salts: Reduced absorption due to increased gastric pH → lower efficacy. St. John’s Wort: May reduce esomeprazole levels via enzyme induction.
